Why Quality Progress Notes Matter
Quality progress notes are an important part of delivering safe, accountable, and person-centred disability support services. They provide a clear record of the support provided, the participant’s progress, choices, wellbeing, risks, incidents, achievements, and any changes observed during service delivery.

Well-written progress notes help support workers, team leaders, coordinators, and management understand what has occurred and what follow-up may be required. They also improve communication between staff, especially when different workers support the same participant across different shifts.

Quality progress notes are also essential for compliance, audits, incident review, NDIS reporting, and service improvement. They provide evidence that services were delivered according to the participant’s plan, goals, and support needs.

Good progress notes should be factual, respectful, timely, clear, and objective. They should avoid personal opinions and focus on what happened, what support was provided, and the outcome for the participant. When completed properly, progress notes protect participants, staff, and the organisation while promoting continuous improvement, accountability, and better quality care.
